Referral Procedures
Given the duties of the referring dentist, given the current GDC guidance, we advise that referring colleagues allow us to establish the Justification for a general anaesthetic, and to take the responsibility of prescribing a general anaesthetic where appropriate.
The Clinic’s policy at the assessment visit is to give a thorough and clear explanation of all anxiety management techniques available, and a clear explanation of the risks involved.
We ask all colleagues to allow us to take the responsibility of prescribing the appropriate anxiety management technique justified according to our protocols.
All dentists referring patients to Queensway Anxiety Management Clinic should have the following documentation. Alternatively, use our electronic referral service through our website at www.anxietymanagement.co.uk. X-rays that are forwarded to us will be returned.

Patient referral form to be sent to clinic
Self-addressed envelope
Please contact us if you require additional supplies of the above.
All NEYO dental patients seen on a referral basis must be accompanied by a written or electronic request, preferably using the referral form provided on our website (www.anxietymanagement.co.uk). Forms can be sent by post or faxed. We prefer, where possible, to assess patients at a separate visit to their treatment appointment. However, we recognise that there will be occasional ‘acute’ referrals requiring more immediate treatment. For acute referrals, patient details can be given by telephone but the referral form must be faxed or accompany the patient to their assessment. Once the patient’s details have been received, they will be contacted to arrange an assessment appointment.
Action – Please use the referral forms provided, enclosing any other relevant records, and ensure patients are aware of the clinic’s location. Patients should bring any prescribed medications with them and be informed that the first visit will be for assessment only. Alternatively, use our electronic referral service through our website at www.anxietymanagement.co.uk. X-rays that are forwarded to us will be returned.
FAILURE TO ATTEND
If a referred patient fails to attend the assessment visit a letter is sent requesting the patient to contact the surgery for another assessment appointment.
If the patient does not respond to this letter, the referring dentist will be informed that we have been unable to contact their patient and have discharged the patient from our care. We are happy to accept a re-referral.
Anxiety &; panic attacks – Health Net
